Our physicians participate in most insurance plans. Here's what to know before your visit.

Payment

Our office requires payment of deductibles, co-pays and past-due account balances at the time of service. If you do not have insurance, full payment is expected at the time of service. We accept cash, checks, Visa and MasterCard.

Using your insurance

Our office will submit claims on your behalf for most insurance companies. Please bring your insurance card to every visit so we can keep accurate information on file, and let us know if your name, address, phone number or insurance plan changes.

Please note: Not all of our physicians participate in the same plans. It is the patient's responsibility to know their coverage. We recommend calling our office or your insurance company to confirm that your provider participates with your specific plan.
